妍慧醫學 x 影像送暖計劃
Imaging modalitieswith full funding計劃資助範圍:
- 超聲波掃描 Ultrasound
- 乳房造影 Mammography
- 電腦掃描 Computed Tomography (CT)
- 磁力共振掃描 Magnetic Resonance Imaging(MRI )
Eligibility requirements 申請者資格
- 持有香港永久居民身份證Hong Kong Permanent Identity Card (HKID).
- 年滿18歲或以上 18 years old or above.
- 持有醫管局轉介信，並新確診癌症分期Referral letter from the Hospital Authority and newly diagnosed cancer for staging.
- 正領取綜援/傷殘津貼 Currently receiving Comprehensive Social Security Assistance (CSSA) or Disability Allowance (DA).
- 一年內沒有參與 ”妍慧醫學診斷中心 x 影像送暖計劃” Has not participated in the " VENUS Health and Imaging Centre x Scan for Hope Program” in the past year.
Required documents所需文件 :
- 香港永久身份證副本 Copy of Hong Kong Permanent IdentityCard (HKPIC)
- 公立醫院轉介信 Referral letter from a Hospital Authority
- 社署綜援申請證明 或 傷殘津貼證明 Proof of Comprehensive Social Security Assistance (CSSA) or Disability Allowance (DA)

領取報告
領取報告，報告會上傳到醫健通。
